Rounds by Haseeb Aslam, M.D.

“The handbook I wish I’d had when I was a resident.” Internal medicine residency doesn’t test what you know. It tests what you can do, fast, while you’re exhausted, covering patients you didn’t admit, with a senior one phone call away. Rounds is the field guide built for exactly that moment. Written by a practicing hospitalist, Rounds is organized the way a good chief resident thinks: recognize the problem, reason through it, take the first move that buys time, and know precisely when and to whom to escalate. It is deliberately practical, the calm, senior voice in your white-coat pocket for the whole of intern year and beyond. Inside you’ll find: Your first week, demystified, how to pre-round, present, write a problem-based note and assessment & plan, call a consult, and function when you’re drowning. High-yield management of the conditions you admit every week, sepsis, pneumonia, COPD, heart failure, atrial fibrillation, DKA, acute kidney injury, GI bleed, stroke, and PE, each updated to current guidelines and cited to its source. Complaint-based bedside workups for the pages you’ll actually get: chest pain, dyspnea, altered mental status, syncope, and abdominal pain. The overnight survival kit, cross-cover triage, routine floor calls (and how not to over-treat them), rapid response versus code, and a clear escalation playbook. Fast references you’ll reach for daily, IV fluids and electrolyte repletion, EKG and ABG made systematic, an empiric antibiotic guide, procedures, anticoagulation, insulin, and a critical-value cheat sheet, plus visual algorithms that condense each workflow to a single glance. Three recurring cues do the teaching. PEARLS capture the ward wisdom that turns a fact into a decision. TRAPS flag the classic intern mistakes and the board-favorite distractors. CALL FOR HELP boxes mark the moment to pick up the phone, because in training, escalating early is a sign of strength, not weakness. Every management recommendation is anchored to a current major guideline, including the Surviving Sepsis Campaign, the ACC/AHA atrial fibrillation and heart failure guidelines, GOLD, the ADA hyperglycemic-crises consensus, ATS/IDSA, KDIGO, and the AHA ACLS guidelines, and cited so you can go straight to the source. Whether you’re a brand-new intern, a senior supervising your own team, a sub-intern, an international medical graduate stepping onto U.S. wards, or a nurse practitioner or physician assistant in inpatient medicine, Rounds is the book you’ll keep in your pocket on call, and recommend to the resident coming up behind you. Recognize it. Reason through it. Act. Know when to call. Get your Rounds. Educational and clinical-decision-support reference for clinicians-in-training; not a substitute for supervision, primary references, or institutional protocols. Always verify and escalate.
